We do not participate with insurance but forms for submission can be provided.

I need to cancel. How do I do that?
To cancel, contact Dawn at (845)-516-4684 or holisticdawnd@yahoo.com. Make sure to cancel over 24 hours before your appointment to avoid a cancellation fee.
Do you offer discounts?
Discounts are offered for payments in cash/check. You may pay in-person at the time of your appointment.
Are virtual appointments available?
Yes, virtual appointments are available via Zoom.
Do you file insurance claims on my behalf?
We do not submit insurance claims directly. We will however provide you with the necessary documentation to facilitate reimbursement through your insurance provider.
How should I prepare for my first appointment?
Please ensure that all completed intake forms are submitted at least 24 hours prior to your scheduled session. Additionally, bring any relevant lab results or diagnostic test reports that may assist in your evaluation.
Can I use an HSA or FSA to pay for services?
Yes, HSA and FSA payments are accepted. Please confirm coverage with your provider.
